Southwest Medical Home Health a home health division of OptumCare / United Health Group offers flexible shifts and the opportunity to make a difference in the care of patients in their home setting.
Primary Responsibilities:
Provides direct care to patients under leadership of the RN
Assures client driven plan of care meets positive outcomes through on going evaluation using evidences based practice
Making certain the home care provided is compliant with State and Federal regulations (documentation and delivered) through participation in chart review and following established process
Majority of job duties are carried out in patient's homes and driving throughout the community
On call, weekends and rotating holidays as needed
Shifts can include weekdays, weekends or combination of both

Required Qualifications:
Current, unrestricted NV LPN license or temporary NV LPN license with the ability to obtain permanent NV LPN license upon expiration of temporary NV LPN license
IV Certification
1+ years of demonstrated competence in acute care, critical care, oncology, HIV / AIDS, advanced illness or medical / surgical patient care setting
Proven ability to lift up to 50 lbs and be able to perform these functions on a regular basis, such as sitting, standing, walking, bending, crouching, twisting and reaching
Willing to travel within Las Vegas providing direct patient care in the home setting
Access to reliable transportation that will enable you to travel to client and / or patient sites within a designated area
Preferred Qualifications:
Home Health experience
Knowledge and competent with wound care and use of wound vac
Nevada Residents Only: The hourly range for this role is $19.47 to $38.08 per hour. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
