The Patient Care Coordinator - Back Office will support the unit in daily clinical operations and administrative support. This role will work with the staff nurses, providers, and team patient care coordinator in patient care coordination and outside scan facilitation.
Responsibilities
Patient Care Coordination
- Responsible for triaging faxes for their disease group
- Triaging faxes includes reviewing incoming faxes, searching for the patient within Epic, express scanning records to the patient's chart when appropriate, and notifying the clinical team that records have been uploaded via email-forward or in-basket message
- Triage records based on urgency and communicate to the clinical team as appropriate or based on need. Follow-up with facility on a regular basis to ensure that records are received in time prior to any relevant clinical visits.
- Manage BOTC shared inbox
- Assists with the financial clearance process as necessary
- Work closely with providers, nurses, and team PCCs to provide safe, reliable, and efficient patient care
Administrative Support
- May maintain contact sheets for specific disease group
- Assist with projects, programs, or systems to improve the overall operation of department
- Assist with onsite mail duties in a rotation; Upload urgent images coordinate with 2nd floor and film room for upload of non-urgent
Qualifications
Required:
- High school graduate or equivalent
- Ability to work independently, organize, and prioritize concurrent responsibilities and demand
- Ability to communicate verbally in person, on the phone, and in writing in a clear and concise manner
- Proficient in the use of word processing (MS Word), email, and data entry in a Windows-based environment
- Demonstrated customer service skills
- Ability to solve complex problems and meet patient needs
- Ability to work in a high pressure, deadline oriented, and complex health care environment
- Ability to review work to identify and eliminate errors and omissions
- Active listening skills.
- Ability to build relationships while maintaining appropriate boundaries.
- Coping skills.
- Ability to identify and triage issues appropriately.
- Attention to detail to ensure scheduling accuracy.
Preferred:
- Bachelor's degree (BA or BS)
- Minimum of 2 years of experience in a patient care setting preferred
- Strong organization skills and attention to detail
- Excellent verbal and written communication skills
- Capable of working in a dynamic healthcare environment, with the ability to prioritize and multitask effectively
- Proficiency in Microsoft Office Suite
- Familiarity with Healthcare Information Software (EPIC)

The hourly pay range for this position is from $26.48 to $37.70 and pay offered will be based on experience and qualifications.
