Logan Health is looking for a dedicated RN Specialty Oncology Navigator to join our team in beautiful Kalispell, MT!
We are seeking a compassionate and skilled oncology professional with expertise spanning the entire cancer care continuum. The ideal candidate will have a deep understanding of clinical oncology practices, cutting-edge therapies, and diagnostic innovations, combined with a patient-centered approach to care, including palliative and survivorship support, family guidance, and addressing the holistic impacts of cancer on individuals and communities.
In this vital role, you will support patients with new, recurrent, or metastatic cancer diagnoses, ensuring seamless coordination of care across our oncology departments, including Hematology/Oncology, Radiation Oncology, Breast Center, and Surgical Oncology.
Key Responsibilities:
Facilitate patient care coordination with other departments (e.g., ENT, Dermatology, Neurosciences, Orthopedics, Primary Care).
Provide education to patients and caregivers, supporting compliance with appointments and treatments.
Collaborate in interdisciplinary tumor boards and cancer conferences.
Triage patient and department calls, assess clinical needs, and present findings to physicians and nurse practitioners.
Work closely with imaging, procedural teams, and external providers to support patients transferring to Logan Health for chemotherapy, radiation, and surgery.
This position offers individualized assistance to patients, families and caregivers to overcome healthcare system barriers. Assists with navigation through the system while providing support, reduced uncertainty and an increase in patient and caregiver’s satisfaction. Counsels, coaches, guides, supports, educates, and interfaces core elements.

Qualifications:
Current Montana RN License or a multi state compact license with authorization to practice nursing within the state of Montana; required.
Minimum of one (1) of the following required:
Bachelor’s degree in a related field and four (4) years experience.
Minimum of six (6) years experience in a related area.
Minimum of five (5) years’ recent specialty experience in a minimum of one (1) of the identified specialty areas required:
Oncology
Surgery
OR Clinical
Knowledge of professional practice standards and regulatory requirements as related to specialty care area required.
Possess and maintain computer skills to include working knowledge of Microsoft Office Suite and ability to learn other software as needed. Knowledge and experience with electronic health records required.
Must function with a high degree of autonomy, strong communication and interpersonal skills. Possess the ability to develop and implement community education, outreach programs/workshops and seminars as defined for assigned specialty area.
Possess ability to comprehensively assess patient’s psychosocial status and demonstrate an understanding of the physiological disease practice as defined for assigned specialty area(s).
Demonstrated ability to work collaboratively with Primary Care teams and/or multidisciplinary care teams.
Possess ability to communicate openly and empathetically with patients and families. Excellent organizational skills, detail-oriented, a self-starter, possess critical thinking skills and be able to set priorities and function as part of a team as well as independently.
Job Specific Duties:
Evaluates the needs and coordinates the care of the patient through the continuum of care by ensuring patient needs are met.
Works closely with members of the multidisciplinary care team and their recommendations. Exhibits professionalism and accuracy in interfacing with providers, clinic staff, patients, and family members involved in the plan of care.
Documents interaction per professional standards related to pre/post care and/or survivorship plan as applicable. Assesses the physical, psychological, social and spiritual needs of the patient and family.
Identifies and assesses the learning needs of the population under care and/treatment. Builds therapeutic and trusting professional relationships with patients, families, and caregivers. Utilizes motivational interviewing techniques to assist patients in meeting goals and managing chronic disease as defined for assigned specialty area(s).
Assists with the organization of the specialty interdisciplinary team meetings and facilitates the team’s recommendations in a professional and timely manner.
Assists with specialty tests per standard and procedure as defined for assigned specialty area(s).
Works collaboratively with all stake holders to identify patient needs and assists in care and services, both medical and social, throughout the continuum.
Collaborates with the navigation team and leadership to optimize processes and metrics to; reduce avoidable ED visits, inpatient admissions, hospital readmissions, cost of care, gaps in care, increase patient satisfaction, clinical outcomes, and provider satisfaction as defined for assigned specialty care area(s)
Assesses education needs, and seeks to update knowledge base to provide quality and current patient specific specialty service line coordination.
Facilitates the use of telehealth and/or self-monitoring technology to enhance the plan of care and patient engagement with the assigned specialty care team as needed.
Actively participates in activities that add value to organizational objectives and workflows, such as process improvement projects and/or community-related initiatives to include community health fairs, workshops, etc.
