Summary

The Advanced Practice Nurse (APN) II Certified Nurse Practitioner (NP) practices in Hematology/Oncology Service to provide a full range of services emphasizing quality, timeliness, and the advancement of the Veterans Affairs (VA) mission. The NP provides advanced clinical practice, consultation, and management. The NP practices to the full extent of their education, training, and certification. The NP has prescriptive authority for a panel of patients or program as indicated by their position.

Duties

• The NP is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients
• Duties include but not limited: Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice
• Develops and implements an individualized plan to achieve optimal population health and disease management
• Demonstrates leadership and works collaboratively with the interdisciplinary team
• Orders and interprets labs, tests, and diagnostic studies
• Prescribes pharmacologic and non-pharmacologic interventions
• Initiates referrals and consults to specialty services
• Provides education to patients and their families
• Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes

• Independently practices as a NP providing a full range of services utilizing treatment guidelines in the delivery of high-quality care
• Collaborates with patients, families, and health care practitioners to develop, implement, and evaluate care
• Utilizes customer service principles, supports veteran self-determination, and serves as a resource for patients and staff in addressing ethical issues
• Demonstrates critical thinking in assessing and managing complex situations
• During a declared pandemic and /or emergency, provider may administer immunizations, evaluate, treat, admit, and discharge within the scope of their training and expertise and completion of any required training
• A provider with experience in the applicable area will be assigned to work the same unit and shift when a provider is detailed during times of crisis for assistance and oversight
• Conducts medical assessment including history, physical and mental status examination
• Formulation of diagnoses and treatment plan for acute and chronic illness based upon exam findings and data collected
• Diagnostic/therapeutic management of acute and chronic gastroenterology and hepatology health problems
• Discharge and/or transfer patient care to appropriate service or medical center after consultation with the receiving provider and/or service
• Initiation and interpretation of lab tests, diagnostic studies, referrals and consultation as indicated Orders FDA approved immunotherapy for GI conditions including recommended surveillance testing
• Provides care for assigned patients
• initiates and adjusts therapeutic regimen as needed
• Select, order and /or administer both non-controlled and Schedule II-IV controlled drug medications within specialty : Adheres to VA Policy and is in accordance with local VA policy and is in accordance with local VA Formulary, State, and Federal Regulations.
