Summary

This position is located at the Lebanon VA Medical Center - Specialty Care Service Line. The Advanced Practice Nurse executes position responsibilities that demonstrate leadership - experience - and creative approaches to management and coordination of complex Veteran care.

Duties

• The Hematology/Oncology ANP is responsible for the assessment - planning - and provision of care for patients enrolled in the Hematology/Oncology program and for inpatients with a Hematology/Oncology consultation
• The ANP is responsible for collaborating with the interdisciplinary Hematology/Oncology team to provide care to a caseload of patients and will support the Hematology/Oncology Physicians in all aspects of patient care including inpatient/outpatient care - telephone consultations - clinical alerts - telehealth - and electronic consultations
• Major duties include - but are not limited to: Assessing the status of Hematology/Oncology patients - planning - intervening - and evaluating indicated actions
• Engaging interdisciplinary team members in improving the treatment plans of their population of Hematology/Oncology patients
• Supporting an atmosphere where caring for Veterans is the foremost concept
• Providing and coordinating quality care to fulfill performance expectations
• Collaborating with the interdisciplinary team to develop - implement and evaluate goals and objectives for Hematology/Oncology
• Facilitating optimal orientation and clinical competence of program staff through preceptor and mentor activities
• Appropriately documents own interventions and reviews appropriate health team documentation of patient treatment plans with the Hematology/Oncology Provider when indicated to treat changes in the patient's condition
• Providing and coordinating quality care to fulfill performance expectations
• Recognizes complex situations that impact patient care and intervenes - using sound judgment - professional attitude - interdisciplinary - supervisory - and appropriate channels
• Participates in establishing policies - procedures - and guidelines to ensure adequate - comprehensive services
• Performs other duties as assigned
