Spartanburg Regional Healthcare System is seeking a full-time Certified Nurse Practitioner or Physician Assistant to join our Oncology Survivorship Program. The APP will work collaboratively with a multidisciplinary team to provide comprehensive follow-up care, symptom management, and wellness support for patients transitioning from active cancer treatment to survivorship.
Position Highlights:
· Monday-Friday 8a-5p with occasional event support after clinic
· No weekends, holidays or call
· Outpatient (Inpatient to provide support to Hem Onc as needed)
· Primary Location: Spartanburg & Pelham (Union and Gaffney as needed)
Responsibilities:
· Survivorship Visits & Clinical Support
· Comprehensive Survivorship Assessment
· Risk-Stratified follow up and shared care
· Psychosocial distress screening and response
· Survivorship Care Plans (SCPs)
· Clinical management and coordination
· Program leadership and accreditation
· Education and communication
· Outcomes and quality
· Documentation and compliance
Minimum Requirements:
· Minimum of 2 years of advanced practice provider experience in Oncology
· Strong clinical assessment and decision-making skills
· Excellent communication and interpersonal abilities
· Commitment to patient-centered care and health equity
