The RN Navigator serves as the primary point of contact and is responsible for coordinating the care and services of the identified patient populations across the continuum of care. The incumbent promotes effective resource utilization and assumes a leadership role with the multidisciplinary team to identify and eliminate barriers to care in order to achieve an optimal clinical outcome for the patient and their caregivers.
Qualifications:
In addition to bringing humankindness to the workplace each day, qualified candidates will need the following:
Three years of experience required with one year in speciality
Bachelor Degree in Nursing required
Registered Nurse License required
Basic Life Support through the American Heart Association required
Nursing Professional Certificaion through the American Nurse Crednetialing Center preferred
Strong psychosocial assessment and clinical skills
Oncology and/or case management, care coordination experience preferred
Physical Requirements Medium Work - exert/lift up to 50 lbs. force occasionally, and/or up to 20 lbs. frequently, and/or up to 10 lbs. constantly
