The role you'll contribute:
Under the direction of the Director of Oncology Support & Education, and in collaboration with the disease specific Physician Program Director; Cancer Care Navigator is responsible for the full continuum of cancer patients from screening to detection, diagnosis, treatment and survivorship follow up. The overall goal is to ensure cancer patients are effectively and efficiently navigated through the health care system receiving appropriate physician, support and educational services through their journey. Responsible for establishing, building and maintaining strategic relationships between our community physicians and their office staff. Responsible for promoting, educating and marketing AdventHealth Cancer Institute Care Navigation Program to HCP physician, community physicians and their office staff.
The value you'll bring to the team:
The focus of this role will be to manage and follow up on incidental lung nodule referrals, high-risk lung nodules found incidentally and from lung cancer screenings.
Facilitates patient continuity of care between hospital departments and provides patient follow-up for laboratory, radiology, and pathology procedures. Timely identification and resolution of issues are handled effectively and in a timely manner; as it relates to the delivery of patient health care and physician relations
Coordinates and participates in weekly thoracic tumor board and lung nodule clinic. i.e., tracks scheduled procedures follow pathology results, coordinates with pathology/radiology, and monitors outcomes of meetings.
Collaboration with ordering providers to coordinate additional workups for patients based on treatment planning- including diagnostic procedures and outgoing referrals.
Understand and follow the Oncology Lung Navigator Workflow Standard Operating Procedure.
Responsible to oversee and track LungRads 3 and 4 results from LDCT scans, collaborating with ordering provider on next step recommendations. Communicates with patients and their caregivers (new or returning) about screening results, follow up appointments, diagnostic procedures, and potential recommendations and/or treatment modalities and what each recommendation means to their specific health situation using the NCCN guidelines.
Understands Cancer Institute Navigation Pathway business model. Explores and reaches out to community physician practices/groups, build relationships to engage those practices/groups, thus growing the regional thoracic inpatient and outpatient market share. In collaboration with the marketing and physician liaison team, promotes the Thoracic Program to community and outreach programs.
Visits physician offices regularly.
Collaborates with other AdventHealth facilities to provide consultation and guidance to expand the Thoracic program.
Enhance patient, community, and provider awareness of lung cancer screening
The overall goal is to ensure cancer patients are effectively and efficiently navigated through the health care system receiving appropriate care throughout the continuum to optimize support, educational services and to provide the communication link for coordination of care among the multidisciplinary health care team. Responsible for establishing, building and maintaining strategic relationships between our community providers and their office staff.
Responsible for working in coordination with Thoracic Program Patient Care Coordinator and the Thoracic Program Nurse Navigator
The expertise and experiences you'll need to succeed:
Minimum qualifications :
- Nursing Required
- 2 years Oncology and/or radiation clinical experience mandatory Required
- Registered Nurse (RN) Required
- Driver's License (DL) Required
Preferred qualifications:
- Bachelor's of Nursing Preferred
- Master's of Nursing Preferred
