The Oncology Nurse Navigator is a professional registered nurse with oncology specific clinical knowledge who offers individualized assistance to patients, families, and caregivers to help overcome healthcare system barriers. She/he serves as a consistent care coordinator ensuring timely access and seamless coordination of services throughout the continuum of oncology care. The nurse functions as a part of the multidisciplinary team providing medical information, education, and shares knowledge regarding internal and external support services and resources. Will be responsible for facilitating: rapid scheduling, coordination of care across service lines and facilities, and tracking patients to ensure completion of recommended treatment plan and surveillance. Will identify and assist patient and families in eliminating potential barriers to care by identifying and referring to appropriate resources. She/he will serve as a clinical resource with expertise in oncology care management. She/he will serve as a liaison throughout the facility and in the community regarding services provided for this patient population. She/he will be an integral part of, and professional member of, the Interdisciplinary team promoting optimal patient care.
Minimum of 3-5 years of clinical nursing practice experience, primarily in area of specialty.
